Gone, if it was going to be saved, it would have been on the hospital train to Barr Yard with the others.

Forgive me if this was already talked about but, what are they saying was the reason for this. Human error or signaling problem?

Human error is what i believe what happened, 1 train rear ended another and fowling the adjacent track in time for another train to strike debris of the prior incident.. The injuries that occured were from when the crew bailed prior to rear ending the stopped train. This could of ended much worse then it did.

Cause code was failure to comply with Restricted Speed. At 44 MPH I would guess so.
